Livescan Background Information *

Given that the nature of our operations are working with kids, our due diligence requires that all volunteers who are working in a capacity directly with the kids need to complete a LiveScan Background Check (finger printed). The LiveScan form and instructions will be emailed to you upon completing this form.

I acknowledge that I will receive an email with the LiveScan form and understand that I will not be able to volunteer until that is completed.
